Dear pet owner, this form provides information about the physiotherapy treatment of your animal. It serves as information for you and for the therapist. In addition to physiotherapy, we apply various therapeutic measures by arrangement. These include: underwater treadmill, biophoton/laser therapy, magnetic field therapy, acupuncture, red light. As a rule, these measures have no side effects. In some cases, however, an 'initial worsening' may occur. These symptoms are the result of increased metabolism due to the treatment and should not last longer than 2-3 days. If unusual disturbances occur in your animal, please inform us immediately.

For optimal therapy success – especially in the case of injuries and post-operative treatments – an interruption of the treatment series should generally not last longer than 14 days.

Agreed treatment appointments must be cancelled at least 24 hours in advance. Otherwise, we will unfortunately be obliged to charge you for the resulting cancellation in the amount of the treatment costs.

You permit the therapists, in the context of your animal's therapy, to collect, process and use your personal data. This is necessary for the provision of the contractually agreed service. You have the right to information about your data at any time. The above declaration also applies to future treatments and can be revoked by you at any time in writing with effect for the future.
